RE:             Signing up with MC prior to performing contract work

In order to perform any contract work with the Housing Authority of Maricopa County (HAMC) for the Neighborhood Stabilization Program (NSP), you must go to this link and register with Maricopa County government:

The following are Terms and Conditions you must agree to in order to register:

·      I am duly authorized to certify and provide the information requested herein.

·      I understand that it is the Vendor's responsibility to update this information when changes occur (addresses, contact information, etc.).

·      If necessary for the completion of this registration, I will submit an IRS Form W9 via fax to the Maricopa County Department of Finance. Information and access to the W9 form will be provided at confirmation.

·      To the best of my knowledge, all information provided herein is accurate and true as of this date.

·      My organization shall comply with all State Statutes, Federal Equal Opportunity and Non­-Discrimination requirements and conditions of employment, in accordance with A.R.S. Title 41, chapter 9, article 4 and Executive Order Number 75-5, dated April 28, 1975.

·      My organization shall comply with all applicable terms, conditions, regulations, laws, policies and procedures set forth in the Maricopa County Procurement Code for my type of procurement (service or commodity).

·      I understand that the online filing of vendor registration supplies information only and does not constitute an assumed obligation by Maricopa County to guarantee contractual awards or agreements to my organization.

·      I understand that Maricopa County intends to use the Federal tax identification number or social security number supplied only as an identifier for tax purposes.
